S8 | E79 Forceps and Birth Injuries with Dr. Giulia Muraca M.P.H. and Ph.D.
Monday Jul 31, 2023
Monday Jul 31, 2023
Forceps assisted birth increases the risk of OASI (obstetric anal sphincter injury) significantly. Dr. Muraca breaks down her doctoral research on instrumental assisted birth to help provide informed consent for everyone giving birth. Being informed can help decrease the risk of injury.
Dr. Guilia Muraca is a perinatal epidemiologist and an Assistant Professor in the Departments of Obstetrics and Gynecology and Health Research Methods, Evidence & Impact at McMaster University.
Her research uses epidemiologic approaches to identify optimal intervention rates in birth (e.g., cesarean delivery rate, induction rate) that minimize the frequency of adverse maternal and neonatal outcomes (e.g., postpartum hemorrhage, obstetric anal sphincter injury, maternal sepsis, neonatal sepsis, birth injury).
Dr. Muraca’s research program at McMaster University aims to produce evidence to support enhanced safety and satisfaction in the care of pregnant individuals and their children, with emphasis on care quality, accessibility and equity.
Dr. Muraca Shares:
- How commonplace are forceps births?
- What is the impact on the pelvic floor and long term issues with prolapse and incontinence after instrumental assisted birth?
- Dr. Muraca’s research has discovered that rates of trauma after Operative Vaginal Deliveries is higher than previously reported - let’s dive into that.
- What are some changes that could be made to make operative vaginal deliveries safer?
- What are your thoughts on our rising cesarean birth rates? Factors at play? Concerns you may have, if any?
- What do you wish more people knew about birth and recovery?
- What are the biggest changes you would like to see made to the current state of obstetrics in Canada?
- Asian-White disparities in obstetric anal sphincter injury: A systematic review and meta-analysis: 39% of women experience fecal incontinence after 3rd or 4th degree tearing (aka Obstetric anal sphincter injury (OASI). Your research showed that there is an increased risk from women who identify as Asian race vs White. Let’s talk about why.

Trigger warning: We discuss maternal death during and after pregnancy, as well as problematic cesarean birth rates.
Dr. Rohan D’Souza is an obstetrician and associate professor with the departments of Obstetrics & Gynaecology and Health Research Methods, Evidence and Impact at McMaster University, Hamilton, Canada. He specializes in the care of pregnant women and gender-diverse people with medical disorders, and his research work is focused on reducing pregnancy-related adverse events. Dr. D’Souza has made it his mission to reduce pregnancy-related adverse events. Dr. D’Souza and Nikki explore the alarming statistics that may be missed and what kind of advocacy is needed to improve outcomes for women during and after pregnancy.
Dr. D’Souza Shares:
- The CBC news article “Canada significantly undercounts maternal deaths, and doctors are sounding the alarm” - got a lot of attention. In your interview, you cited the importance of better maternal health monitoring systems to prevent avoidable deaths. The article stated that Canada undercounts the maternal mortality rate. Let’s talk about that.
- Let’s talk about the increasing cesarean rates. We know they vary tremendously from provider to provider and hospital to hospital. What is the current data and trend over time? What kind of impact do you believe this trend has on maternal (in the short and long term?)? We know the age of a first-time mother is increasing, as well as body mass index - but is there something else going on here?
- The lost skills of breech birth and forceps and how that is impacting C-section rates
- What we need to know about the ARRIVE trial and how the data showing induction at 39 weeks reduces C-sections has been misinterpreted by many

During the COVID-19 pandemic Natalie found herself being Canada’s 29th case… Her isolation period gave Natalie an even wider depth of insight around how to deal with situations that are beyond our control and the trauma it can play on us mentally. Her positive outlook by searching for silver linings through gratitude and mindset shifts, is something that she hopes she can help others with as they navigate the new and challenging environment that we live in today.
Natalie is the mother of 4 boys and made it her mission to give children the tools needed to meditate. Natalie created MediTATE the meditation bear and she’s breaking down how meditation benefits children of all ages.
Natalie Shares:
- Hearing versus feeling (why is it important PRACTICE mental fitness, calmness, deep breaths...) and understanding this in terms of physical activities vs mental activities.
- Expectations around meditation in children -- what is the goal here and why meditation/mindfulness is actually a language that is easier taught when you are young!
- All about mediTATE (TATE) and how it is completely different from any other tool that exists on the market for children's mental fitness. This includes:
a) The content (what types of meditation stories will you get with miindfully)
b) The fully sensory experience (olfactory, auditory, visual, tactile) and why this is essential to help children engage in learning about their "inside body",
c) Why having a friend who is unique to YOU, is a wonderful way to feel grounded, loved, calm and centered. Especially in times of isolation.
